*1. Passport-sized photo
A passport-sized photo taken recently of the applicant.
*2. A Photocopy of valid passport
The passport must be valid for more than 12 months.
*3. Certificate of HSK
Certificate of HSK (Chinese Proficiency Test),No need for English medium programs
*4. Bachelor graduate certificate
According to Liaoning Province Government Requirement: Graduation certificate in languages other than Chinese should be translated into Chinese and be certified by notarization. If you can not find translation organization, please contact CUCAS.
*5. Undergraduate academic transcript
According to Liaoning Province Government Requirement: academic transcript in languages other than Chinese should be translated into Chinese and be certified by notarization. If you can not find translation organization, please contact CUCAS.
*6. Two Letters of recommendation
Must have the signature of professor or associate professor.
7. Other certificates or proof such as a certificate of award or a certificate of scholarship
8. Proof of recent work or proof of no criminal record(After graduation more than 12 months)
*9. Physical Examination Form(only valid for 6 months)
Please take it to hospital to have a checkup with all items, including personal information, photo, checking date, doctors' suggestion and signature.
10. Supporting Documents: Certificate of Good Conduct
Provided by your current/previous school/company in Chinese/English to improve your success rate.

For applicants who are in China: If you are an employee, you still need to provide employment separation certificate and your current residence permit.
